This form is part of Minnesota Statutes Section 523.23.

See Subd. 3 Requirements, which in part said (when we made this entry): "To constitute a statutory short form power of attorney," as this phrase is used in this chapter the wording and content of the form in subdivision 1 must be duplicated exactly and with no modifications, parts First, Second, and Third must be properly completed, and the signature of the principal must be acknowledged."

   Power of Attorney Form (Oppenheimer Wolff & Donnelly)
Empowers another as attorney-in-fact to execute Forms 3, 4, and 5 in accordance with Section 16(a) of the Securities Exchange Act of 1934 for an officer or director of a company.
